In collaboration with the City's Police Services and Public Works Departments, the Community Outreach Worker provides individuals experiencing homelessness with case management services, access to available resources, and develops permanent housing plans. Through intensive case management, our community outreach worker offers services including:
- Housing vouchers/rapid rehousing programs/and case conferencing
- One-on-one engagement to build rapport and trust
- Food and clothing referrals
- Transportation assistance
- Family reunification and relocation to support systems in other areas
- Emergency shelter placements
- Medical and mental health care referrals
- Financial literacy information
- Social services assistance
- Substance abuse treatment referrals
- Homeless prevention
